The AQD Electronic Reporting Registration form must be completed once for each facility wishing to use SLEIS for electronic reporting. The AQD Electronic Reporting Signatory Application form must be completed once for each user who will be responsible for actually electronically submitting reports to AQD (e.g., the responsible official).

After you have completed the required forms, mail them to the address indicated on the form.
